1. Fill out the REQUEST FORM and CONSENT FORM then return to Bakersfield Pediatrics.
Make sure to use the email you want connected to your Patient Portal account.

2. Receive Security Token
Once your application for Patient Portal access is approved, you will receive a call from the practice to pick up your token in person at one of our offices.

4. Validate and create account.
Once your account has been validated you can begin enrollment.
